Help a Brother Out is a Story Mission in Red Dead Redemption 2 (RDR2). This is one of the "Main Quests" of the game and players must complete it to advance the Story. Completing Story Missions opens up Stranger Missions as well as additional content throughout the game.

In the west of Saint Denis, the monk, Brother Dorkins can be found asking for donations. Donating to him increases your honor. He takes you to a Fence shop and you then examine with a red book on the shelf. Make your way down and free the captives.
Return to Brother Dorkins, he then offers you payment but by not accepting it, your honor increases.
