A Fork in the Road is a Story Mission in Red Dead Redemption 2 (RDR2). This is one of the "Main Quests" of the game and players must complete it to advance the Story. Completing Story Missions opens up Stranger Missions as well as additional content throughout the game.

At a random point in the town of Saint Denis, Arthur Morgan (you) starts to cough blood abundantly, and eventually falls to the floor and passes out. You're wake up momentarily being dragged by your feet, by a stranger, then pass out once again. When you come to, you find yourself struggling to stand but being guided by the same stranger to a doctor. Stumble your way to the entrance then the man will point you into the door.
Speak to the lady in the cubical and she'll call a doctor for you. The doctor will come out immediately and bring you to his office. Once there, sit down and do as the doctor says. When you ask him what the problem is, he'll tell you that you have been diagnosed with Tuberculosis. He gives you his condolences then a shot to give you some energy.
You walk out of the office slowly, and are instructed to leave the area. Do so and on your way, you'll remember conversations you had with people throughout your journey so far. Once you leave the area, you'll assume normal control of Arthur once again, and the mission will end. However you'll be put into a sick state which negatively affects the way you process consumed food and drinks.
Notes & Tips
- The final quote that narrates in Arthur's mind is that with Mrs. Downes, about how he basically killed her husband. If you cast your mind back, Mr. Thomas Downes was very sick and coughed on Arthur when he was being intimidated, a common way to spread Tuberculosis.
